Community Health Works serves as the Outreach and Education Technical Assistance sub-grantee for the Georgia Health Information Technology Regional Extension Center, which is administered by the National Center for Primary Care at the Morehouse School of Medicine. We are working with physicians across our region to provide outreach, education, and technical assistance for practices transitioning from paper-based practices to electronic health records, as well as, practices working to achieve meaningful use incentive payment. Through this program, CHW provides HIT unbiased support services to 800 medical professionals through the 23 counties of these regions.
CHW provides support in the selection and implementation of electronic health record systems (EHRs) as well as hardware and support services, and is working closely with the IT consulting community to operate a preferred IT consulting group that consists of screened, qualified IT consultants with extensive experience in supporting EHR based medical practices. Further, CHW is working closely with the only functional Health Information Exchange (HIE) in Central Georgia, Central Georgia Health Exchange (CGHE), to network as many providers as possible, which will allow providers to share Continuity of Care Documents (CCDs) and other health information between practices and The Medical Center of Central Georgia.
